Build an exception review queue
Give uncertain or failed work a destination your team can actually manage.

The lesson
An exception is not a failure if the team can see and resolve it. Create a queue for missing fields, conflicting records, low-confidence interpretation and actions the agent is not authorised to take. Each card should show the source, proposed action and reason it was held.
Reviewers need simple choices: approve, correct, ask for more information or reject. Capture those choices so you can learn which rules need adjusting. Track queue volume over time; a growing queue may mean the automation is moving work rather than removing it.
